A plastic surgeon could tell you for sure. I suspect it would still be considered cosmetic, but it might be worth checking out.

      One breast smaller

Just thought I would put in my two cents on this topic. I too always had a larger left breast (first time I have heard about this correlation). To compound the situation, in the mid-90s I had several large breast biopsies done on my RIGHT breast so I became reallllly lop-sided. I finally decided to do something about it and had a breast reduction in 2005. The surgeon brought me down to a comfortable C-cup, removing a total of 5 lbs. (!) of breast tissue. Best thing I ever did - no regrets.
I don't know if implants would be covered, but I WAS able to have the reduction covered by insurance due to scoliosis, back pain, etc. Just thought I would pass that on.

This makes me think of an interview I read yesterday with Adrienne Curry, she's the model who won America's next top model(first season) and she just had surgery b/c she had two different breast sizes. One was a large A cup and the other a small C cup. They are now two large C cups and she says it hurt and will for a while still.

She doesn't have scoliosis and has modeled for years with her breasts being different sizes, she even did nude shots but she always stood a certain way or put her arm up to cover the difference.
