We are pleased to invite all researchers, medical and health care professionals, and patients interested in conservative management of scoliosis and related spinal deformities to join us in Boston to explore all aspects of this field. Well known clinical and scientific experts from around the globe will present the current state of the art for conservative care of scoliosis patients. This is an unprecedented opportunity to learn what is available and why it is so important.

Researchers, Medical and Health Care Professionals, and Patients are welcome to attend our Education Session (1 day) and/or Scientific Program (2 day)

Clinicians and Orthotists who attend the Scientific Program and want to learn more about the Concepts, Construction and Certification Requirements for various Braces are also able to attend an additional Clinic Day (1/2 day)

This PDF allows typing directly onto the form. Please click on the typewriter function and fill in the fields. An X should be put in the spaces that you are checking off such as MD, or to indicate which credit card you are using. After you type in all of the fields you can save the form under a different file name and then email it as an attachment to ageorge@scoliosis.org if you are paying by credit card.

Alternatively, you can print the form and either fax it to 781-341-6333 if you are paying by credit card, or mail it with a check to the National Scoliosis Foundation.
